What Should I Do Before My First Appointment?

Please fill out the patient forms under “Patients” to ensure we can dedicate all our time to your treatment. If you are unable to fill the forms out online, please arrive 15 minutes early. If we are coming to you, please note we will need advanced notice if you need help completing your forms.

What should I bring to my First Appointment?

  • Photo ID
  • Insurance Card (if applicable)
  • Referral from MD/Other Provider (if applicable)
  • Your Payment Method (Credit Card, Cash, Check, HSA/ FSA Card)

What Should I Expect During My First Appointment?

  • We will talk about why you want or need physical therapy and your goals
  • Then your physical therapist will evaluate you for 20-30 minutes using physical therapy tests to find the source of your problem(s)
  • Measurements will be taken along with education regarding your pain and/or injury
  • Then we will have about 15 minutes to start your first round of manual therapy interventions (soft tissue massage, joint mobilizations, muscle energy) and may give you an initial prescribed exercise program

What should I Wear?

Loose, comfortable clothing and supportive athletic shoes that can:

  • Easily expose the part of the body we will be focusing on.
  • Allow you to move and exercise without limitation.
